When Inquiries Become the Solution
Seventy-two people died in the Grenfell Tower fire in June 2017.
An inquiry was announced almost immediately.
Eight years later, it is still ongoing.
That delay is usually framed as responsibility. As care. As seriousness.
But it’s worth asking a quieter question:
What, exactly, are inquiries doing once they begin?
Because if inquiries were the mechanism through which accountability reliably followed, this would have ended very differently.
When a scandal breaks, the public demand is simple:
Someone must be held accountable.
The institutional response is just as predictable:
We’re launching a full, independent inquiry.
That announcement does two things at once.
First, it signals gravity. It tells the public the matter is being treated seriously.
Second, it defers consequence. Because once an inquiry exists, nothing substantive is expected to happen until it concludes.
The inquiry itself becomes the action—not the precursor to action.
Speed as Signal
Inquiries are announced quickly. This isn’t accidental.
Speed shifts the narrative from “What are you doing?” to “An inquiry is underway.”
Pressure releases. Attention moves. The sense of urgency softens.
Grenfell. Infected blood. COVID. Different scales, same response.
The faster the inquiry is announced, the longer it can run without appearing slow.
Thoroughness as Delay
Inquiries are framed as needing to be comprehensive, independent, and rigorous.
All reasonable qualities.
All qualities that guarantee time.
“Thorough” means years of evidence.
“Independent” means distance from political urgency.
“Rigorous” means phases, hearings, submissions, reports.
The Chilcot Inquiry into the Iraq War ran for seven years.
The Infected Blood Inquiry took seven years.
Grenfell’s second phase is still ongoing.
The more thorough the inquiry, the further accountability is pushed into the future.
Findings Without Force
Public inquiries can establish facts. They can expose failures. They can make recommendations.
They cannot prosecute.
They cannot compel reform.
They cannot enforce consequences.
Reports are published. Recommendations are “noted.”
Implementation is reviewed, delayed, diluted, or quietly abandoned.
The inquiry concludes. The structure remains.
Time Does the Work
The delay isn’t a side effect. It’s the mechanism.
During a five-to-eight-year inquiry:

The emotional weight dissipates
By the time findings emerge, they confirm what was already known—but without the force of the original moment.
The report becomes a historical document, not a trigger.
From Trigger to Reference Point
Once complete, inquiries are rarely acted upon.
They are referenced.
“We take the findings seriously.”
“Lessons must be learned.”
“We’re reviewing the recommendations.”
The inquiry’s existence becomes proof that the system responded—regardless of whether anything changed.
Even fast inquiries can be ignored.
Leveson made clear recommendations on press regulation. Most were shelved.
The problem wasn’t speed.
It was enforcement.
What Inquiries Actually Do
This isn’t about bad intent or hidden coordination.
It’s about function.
Inquiries serve three structural roles:
Pressure release – the announcement diffuses outrage.
Temporal distance – delay ensures urgency expires.
Accountability theatre – process substitutes for consequence.
Inquiries aren’t designed to fail.
They’re designed to absorb.
They absorb anger, attention, and demands for immediate action—and convert them into years of process.
The Pattern
Crisis happens.
Inquiry announced.
Pressure releases.
Time passes.
Report published.
Recommendations noted.
Minimal change.
Next crisis.
If inquiries worked as mechanisms of accountability, we wouldn’t need so many of them.
If consequences followed automatically from findings, the same failures wouldn’t keep repeating.
The uncomfortable question isn’t why inquiries take so long.
It’s why we keep treating delay as diligence.
Because once you see the pattern, it’s hard to ignore the possibility that the delay isn’t a flaw in the system—
it’s the system.
